A study by Tinker et al (2026) reported some great findings, including:

A clear association between upright positioning during the active pushing stage and of labour & physiological birth rates.

More specifically, for every 10% increase of time spent in upright positions during the active pushing stage of labour, the likelihood of physiological birth increased by 26%

Membrane sweeps, something routinely offered to everyone at 39 weeks in the UK, so they are well worth considering ahead of this choice.

A membrane sweep involves a vaginal examination where a midwife or doctor gently separates the membranes from the cervix. This can stimulate prostaglandins and may help encourage labour to begin naturally.

Possible benefits:
• May increase the chance of spontaneous labour starting
• May reduce the likelihood of needing a formal induction later on
• A less medical way to try to encourage labour

Possible downsides:
• Can cause discomfort during the procedure
• Bleeding or spotting afterwards
• May prompt irregular contractions or a longer latent phase of labour
• Small increased risk of infection
• Uncommon but it is possible to accidentally rupture the membranes (waters breaking)
• It doesn’t always work, estimates suggest around 1 in 8 sweeps lead to labour starting

Some people also find that interventions like sweeps can start the “ticking clock” feeling towards the end of pregnancy, which can affect how they experience the final days or weeks.

As always, it’s your choice. Understanding the evidence can help you decide whether to accept, decline, or wait.

If you do choose to have a membrane sweep it can be really helpful to be active prior, helping your baby into a good position in preparation, will ultimately help give you the best chances of your membrane sweep being successful. Curb walking, birthing ball exercises, pregnancy yoga, just walking to the appointment even.

Vaginal examinations (VE’s) are optional in labour 🤍

As midwives, we perform them often and they are a routine, clinical skill for us. There is nothing awkward, nothing embarrassing, and nothing “too much” about your body. It’s simply part of our work, and we approach it with professionalism, respect, and care.

✨ Common reasons a VE may be offered
• As part of the induction process
• To assess how dilated you are, they are routinely offered every 4 hours in established labour
• When making decisions about pain relief (like an epidural)
• If there are concerns about baby’s wellbeing

✨ Pros:
• Can give information about labour progress
• May help guide decision-making
• Can sometimes provide reassurance

✨ Cons:
• Can feel uncomfortable or invasive
• Doesn’t always predict how quickly labour will progress
• Can provide disappointment even though they aren’t definitive
• Increased risk of infection (especially after waters have broken)
• May interrupt your labour flow

Consent always comes first. You can ask questions, you can say no, and you can change your mind at any time. Just because you choose to have one examination, doesn’t mean you consent to all.

You should never feel uncomfortable or embarrassed, this is our everyday work, but it’s your body and your birth. You deserve to feel safe, informed, and respected every step of the way 🤍
