Social Prescribing Care Coordinator
Hours: 37.5 per week
Location: Southport Community Centre (covering Southport & Formby PCN)

Make a real difference in your community.

We’re looking for a motivated and compassionate Social Prescribing Care Coordinator to join our Southport and Formby Social Prescribing Team.
As the first point of contact in to the service your role will be pivotal in supporting good quality referrals from GP practices, telephone triaging clients to ensure their understanding of Social Prescribing, identifying any clients that may need to be prioritised and making instant referrals where appropriate.

The Social Prescribing Care Coordinator will work closely with the Social Prescribing Link Workers and other primary care professionals within the PCN to support the management and throughput of the social prescribing referrals.

A Massive Thank You to Alpha Taxis! ✨

This week we’re absolutely thrilled to celebrate another local business, Alpha Taxis, who’ve just stepped up in the loveliest way to support our work and everyone who uses our community centre.

Thanks to their kind donation, we’re able to keep improving our Activities programme but, even better, they’ve kindly provided another big stack of sturdy Bags For Life that will be used every week by our Volunteer Shoppers and in our popular Healthy Cooking Course!

Little things like this make a HUGE difference. Reusable bags mean less waste, easier carries for our volunteers, and happier participants leaving our courses with their homemade meals.

This is proper community spirit in action: local business + local charity = stronger, kinder, healthier neighbourhood.

So next time you need a taxi (and let’s be honest, we all do when it’s pouring down!), please think of Alpha Taxis. They’re not just driving people around town, they’re helping look after the people in it.
