Serving 70 residents on our Bethlehem campus, we offer personalized, compassionate support to help long-term and short-term rehab residents thrive and recover. With over 50 years of experience in skilled nursing care, the Leader family has guided our commitment to providing exceptional resident care. At Country Meadows Skilled Nursing and Rehab Center, we offer both continual clinical care and short-term support during recovery, ensuring our residents receive the compassionate and comprehensive care they need to lead a quality life. Located in the Lehigh Valley on our Bethlehem campus, our center is home to only 70 residents, allowing us to maintain above-average staffing ratios and provide personalized attention. Our dedicated team of co-workers collaborates to support all aspects of wellness, addressing not only medical and therapeutic needs but also the emotional and spiritual well-being of our residents. Long-term care residents enjoy a vibrant daily life filled with activities, social events, spiritual enrichment, meals and outings. For short-term rehab residents, we conduct thorough assessments upon admission to create individualized care plans. Our team regularly reviews these plans and discharge goals, focusing on what truly matters to the resident, their family and caregivers to support their progress. While we are proud of the numerous awards we have received for quality care, staffing levels and our highly acclaimed restorative nursing program, our greatest reward is witnessing our long-term care residents thrive and our rehab residents recover.

Meet the Team 💙

Say hello to Connie Quier, our amazing Director of Dynamic Living! Connie’s journey with Country Meadows began 26 years ago in Allentown, and after working at several other senior living communities (and collecting a few impressive certifications along the way), she’s come full circle—right back where her career began.

Connie is a certified powerhouse in senior engagement, holding credentials in Occupational Therapy, Dementia Care, Montessori Training, and more. She even received the Pride Award from the Alzheimer’s Association for her incredible contributions to the cause.

When she’s not leading life-enriching programs on campus, Connie’s all about staying active and having fun. She’s run a half marathon (and is already signed up for her next one 🏃‍♀️), loves traveling and motorcycle rides with her husband of 45 years, Glenn, and treasures time with her two daughters, two grandkids, and her pups, Gunner and Kimber.

Meet the Team 💙

As Director of Maintenance at our Bethlehem Nursing and Rehab Center, Moses plays a key role in keeping our campus running smoothly and safely. He joined Country Meadows after learning about the organization’s strong reputation and has since become an integral part of our team.

Moses brings a wealth of experience and education to his role, holding certifications and diplomas in Fire Safety and Emergency Preparedness, Digital Literacy, Electronics, Electricity and Computer Hardware Repairs, among others. His dedication ensures that our residents and co-workers enjoy a safe, comfortable and well-maintained environment every day.

When he’s not at work, Moses enjoys staying active—spending time at the park, hiking, playing ball and visiting the zoo with family and friends.

⏳ Wyomissing Campus Spotlight 🌟

We’re excited to celebrate Wyomissing campus co-worker, Navia Bourland, who participated in this year’s Holiday Card Co-worker Art Contest! Her piece, “Timeless,” earned the People’s Choice Award, voted upon by peers company-wide through our internal channel Talk and by fans across our social media. 🏅👏

Navia shared the meaning behind her artwork:

“My artwork is an hourglass that shows we start off as children, and over time we get older. As life progresses we learn that spending time with others is one the most important values of life. We all go through the stages of life from youth to elderly. Unfortunately life is way too short, and we are grateful for the time we get to spend it with family, friends, and colleagues. Working at Country Meadows inspired me by being able to communicate with residents and hear their stories from their childhood to present is very heartwarming. Being able to share fun times and good memories that will last forever is cherished.” 💛
