If you wish to train at a U.S. based, ACGME accredited institution, you should begin your preparation by learning more about the rigors and challenges of medicine in the U.S. The only way to do that is by rotations and observerships in U.S. based clinical sites, where preceptors can help you adjust to the differences that residency program directors will expect you to be able to navigate. Clinical Experiences, we offer Foreign Medical Graduates the opportunity to learn in NYC-based clinical rotations for competitive rates. Besides the invaluable experience that comes with our clinical rotations in the U.S., we provide proper mentorship to our students to help them maximize their potential. As a cancer doctor, I see the dangers of microplastics every day. We are constantly drinking water from plastic bottles, and it’s not good for us 🚫🥤⁣

Being a doctor in American movies is VERY different from being a doctor in real life 🥼🇺🇸⁣

You can’t just go to a patient and say “You have 3 months to live, please make sure your will and other documents are in order” 🤦‍♂🙅‍♂️❌⁣

Imagine the condition of the patient! Have some humanity, and don’t give them timelines that even you’re not sure of! 🚫⁣

This empathy is part of American culture, and IMGs from various parts of the world are not used to this kind of an environment... so they end up treating this delicate situation as if it’s an administrative task 🤷‍♂⁣

Program Directors HATE this because as residents you are expected to treat your patients with respect and utmost kindness ✅⁣

It’s YOUR JOB as a doctor to make your patient feel safe with you, trust you, and want to get treated by you! 👨‍⚕👩‍⚕⁣

You can’t just wheel over a patient when moving them from the general floor to the ICU! 🏥🚨⁣

You need to write a Transfer Note 📝⁣

This is one of the most critical moments in hospitals because the number of times we’ve seen patients ‘disappear’ because a Resident ‘forgot’ to write a Transfer Note is scary 🫠🤷‍♂⁣

You are changing their “Level of Service” - and the new team needs to know exactly when that happened and why:⁣

⬆️ Floor to ICU: Did the patient get worse? Why do they need critical care now?⁣

⬇️ ICU to Tele: Are they stable enough to step down? What is the new plan?⁣

This is the FINAL STEP of one of the most important things you need to learn as a doctor in America (writing patient Notes!)🥼🇺🇸⁣

While the Discharge Summary is for the doctors' information, the Discharge Instructions are for the patient. This document ensures your patients stay healthy after they leave your care! 🏡⁣

You have to verify the practical details:⁣

💊 Medications: If you prescribed medications, did the patient actually get them? Do they know how to take them?⁣

🥗 Diet: What exactly should they eat or avoid?⁣

📅 Follow-ups: Which doctors do they have to follow-up with, and when?⁣
