Welcome to Evolution Physical Therapy and Wellness! We are a small outpatient Physical Therapy clinic working within the South Yuba Club. We take full advantage of the gym setting to help promote healing and strengthening for our patients. We truly believe our patients come before us and that their care and the customer services they receive is what sets us apart. We encourage humor, laughing and smiling as part of the healing process. Working to make our patients feel more comfortable and relaxed so they can enjoy their journey and Evolve to a brighter, smarter tomorrow.

We are looking for a committed Physical Therapist to join our team, and to act as the patient’s partner throughout the Evolution of restoring health and ensuring the return to their best self.

Responsibilities:

Identify and meet patients’ goals and needs.
Offer cost-effective treatments that help improve clients’ motion and mobility.
Reduce the need for medications and provide alternatives to surgery.
Develop care plans using a variety of treatment techniques.
Create fitness- and wellness-oriented programs tailored to patients’ specific needs.
Provide quality, personalized and evidence-based care and proven interventions.
Motivate patients during treatment in order to help them function optimally.
Promote clients’ healthy lifestyle by improving strength, flexibility, balance, and coordination.
Consult and practice with other health professionals if needed.
Evaluate effects and monitor and communicate progress.
Document patient care services.

Skills:
Valid license to practice physical therapy; PT or PTA license.
Proven work experience as a physical therapist or physical therapist assistant.
Ability to manage patients with different types of personalities.
Current knowledge of treatment practices.
Drive to continually learn and grow.
Excellent interpersonal communication skills.

Graduate degree or greater in physical therapy'.

Evolution is coming to you on this rainy day to say Thank You for all your continued support through these times and over the last year, as May 1st Marked our One Year Anniversary!!

We are open to see patients and continue to expand our hours to meet your needs. Over the last few months we are all learning how to EVOLVE and with that we are taking extra precautions and sanitizing between every patient. We are only seeing one patient in the office at a time and ask that everyone including staff wear a mask while here. In addition to washing your hands before appointments and in between patients.

We hope all are staying Healthy, Happy and as Active as possible.
