Pelvis NYC The Go-To-Center for Orthopedics & Male Pelvic Floor Therapy in New York City

At PELVIS NYC, we take an individualistic approach to treating pelvic floor dysfunction in men. The main goal for treating pelvic floor dysfunction is to relax the muscles and provide minimally invasive treatment using manual physical therapy, technology and exercises to regain your pelvic health. Pelvis NYC is also created as a platform where men can share their experiences without any judgment.

Most pelvic floor work isn’t really about the muscle… it’s about the brain’s response to what’s happening.

A lot of clinicians get stuck in theory; the “clock method,” vague pressure points, random releases and struggle to translate that into real change.

If the brain feels unsafe or confused, the pelvic floor won’t budge.
If the brain feels clear and supported, tone drops.

In this video, I’m showing how simple soft-tissue work in hooklying can create immediate shifts when you target the nervous system, not just the anatomy.

This is the difference between:
aimless poking & intentional input that actually changes symptoms.

Pelvic rehab should be specific, informed, and effective.

If you're in the middle of a PGAD flare-up, I know how intense and exhausting it can feel. Sometimes, just getting a moment of relief makes all the difference. Cold therapy — like gently placing a wrapped ice pack on the area — can help calm nerve endings and reduce the overwhelming sensations. Some people also find relief using topical anesthetics like lidocaine to numb the area for a little while.

These aren’t long-term fixes, but in tough moments, they can help you breathe again. Please be gentle with yourself, and check in with a trusted healthcare provider before trying anything new.

Small lifestyle changes can make a big difference in managing Persistent Ge***al Arousal Disorder (PGAD) symptoms.

While PGAD is complex, pelvic floor physical therapists often recommend supportive daily habits that help reduce discomfort and improve quality of life.

Lifestyle tips a PT might suggest:

🧘‍♀️ Mindfulness & breathwork to calm the nervous system

🚶‍♀️ Gentle movement (like walking or yoga) to improve blood flow and reduce tension

🪑 Postural awareness & ergonomic changes — avoiding long periods of sitting, using cushions, or adjusting your workspace

👖 Avoiding tight clothing around the pelvic area

🌙 Sleep hygiene & stress reduction techniques to support hormonal and nervous system balance

💧 Staying hydrated and maintaining regular bowel habits to avoid straining and pressure on pelvic nerves

These daily habits, combined with pelvic floor therapy, can play a powerful role in reducing PGAD flare-ups.

Managing Persistent Ge***al Arousal Disorder (PGAD) requires a holistic, multidisciplinary approach that addresses both the physical and psychological aspects of the condition. The goal is to reduce symptoms, improve quality of life, and support overall well-being.

🔹 Medications (like SSRIs and SNRIs) help regulate mood and reduce unwanted arousal sensations.
🔹 Pelvic floor physical therapy can ease muscle tension, improve circulation, and calm irritated nerves.
🔹 Cognitive-behavioral therapy (CBT) helps manage anxiety and reframe distressing thoughts.
🔹 Mindfulness-based therapies promote stress reduction and emotional balance.
🔹 Alternative treatments like acupuncture or massage may offer added relief.

A personalized treatment plan created with a healthcare provider is key to effectively managing PGAD.

Physical Therapy for PGAD – What It Really Does

Pelvic floor physical therapy is one of the most effective non-invasive ways to manage Persistent Ge***al Arousal Disorder (PGAD). Here's how it works:

🔹 1. Muscle Relaxation
Many people with PGAD have tight or overactive pelvic floor muscles, which can worsen nerve sensitivity.
Manual therapy (hands-on techniques) helps release this tension, improving blood flow and easing symptoms.

🔹 2. Nerve Desensitization
PGAD is often linked to irritated nerves — especially the pudendal nerve.
Nerve gliding exercises and gentle stretches help free up the nerve and reduce its hypersensitivity.

🔹 3. Biofeedback Training
Biofeedback uses sensors to show how your pelvic muscles are working in real time.
It helps patients learn to properly relax and control these muscles, which is key for reducing PGAD flare-ups.

🔹 4. Relaxation & Mindfulness Techniques
Stress and anxiety can trigger or worsen PGAD symptoms.
Diaphragmatic breathing, progressive muscle relaxation, and mindfulness practices help calm the nervous system and reduce symptom intensity.

Pelvic floor PT addresses both the physical and nervous system components of PGAD — making it a powerful tool for symptom management and long-term relief.

The pudendal nerve is like a message highway between your ge****ls and your spine. It helps you feel sensations in that area.

When part of this nerve — especially the branch that goes to the p***s or cl****is — gets pinched or irritated, it can send false signals to your brain, making you feel aroused even when you're not.

This nerve problem can be one of the causes of PGAD (Persistent Ge***al Arousal Disorder).

In some cases, releasing the pressure on the nerve has actually helped people feel much better.
