Not everyone is ready for a full-time in-home aide, and that’s okay.

COHME’s Specialized Dementia Support Program is a stand-alone service that can be used whether or not you receive COHME aide services. It’s especially helpful for families in the early stages of dementia who want education, reassurance and a clear plan.

The program includes:
• A comprehensive initial assessment, with follow-ups every week
• Ongoing support from a COHME social worker by phone, email and in-person as needed
• Guidance, referrals and resources for support groups, therapy, and further education
• Training for family caregivers and home health aides to strengthen day-to-day care

Support that helps families feel informed, prepared and confident — without the commitment of a full-time care schedule.

Talk to our team to determine if it's right for you. You can reach us at: https://www.cohme.org/contact.

Starting home care can feel overwhelming, but it doesn't have to be.

At COHME, we guide families every step of the way:
• Onboarding: getting to know your loved one and their routines/needs
• Care plans: personalized strategies tailored to needs and preferences for the whole family
• Communication: regular check-ins to keep families informed and confident

Our goal is simple: to make the transition smooth and supportive so you can feel comfortable and cared for from day one. 💙

Caregiving is often described as rewarding (and it truly is) but it’s also emotional, exhausting and sometimes overwhelming.

You might feel guilt for needing a break, or notice how your relationship has shifted as roles reverse.

Decision fatigue can sneak up, leaving you questioning whether you’re doing the right thing.

These feelings are normal, and you are not alone in experiencing them.

At COHME, we see the dedication caregivers give every day. That’s why we offer guidance, resources, and hands-on support to help you navigate challenges and take care of yourself along the way.

Because caring for someone else starts with caring for yourself, too. 🩵

Aging in place isn’t just staying in your home, it’s living safely, comfortably and independently for as long as possible.

With the right care and guidance, older adults can maintain daily routines, stay connected to the people and places they love and enjoy a meaningful quality of life. Key considerations include home safety, access to care and personalized support plans.

At COHME, we help families understand how aging in place is realistic and provide guidance to make it safe, manageable and meaningful.

Consistency matters in home care.

That's why we prioritize continuity by matching clients with caregivers they can build trust and familiarity with.

Because strong relationships lead to better care. 💟

In dementia care, engagement isn’t about “keeping someone busy.”

Meaningful engagement supports connection, identity and quality of life and it’s most effective when it’s rooted in a person’s interests and life experiences.

With our Dementia Support Program, these experiences are thoughtfully planned and supported (not random!) because the right kind of engagement can make everyday moments feel more meaningful.

Dementia care doesn’t only happen during our scheduled visits or check-ins, it happens in the moments in between.

In the evenings when routines feel harder.
During transitions that suddenly cause confusion or agitation.
When you’re unsure how to respond in the moment.

COHME’s Dementia Support Program is designed to give you the tools, knowledge and practical strategies to navigate those situations with more confidence, even when we’re not there. Through in-home coaching, personalized guidance, and ongoing support, we help you feel prepared for the day-to-day realities of dementia care.

Because the moments you manage on your own are often the ones that matter most.

International Women’s Day is a moment to recognize the women who care, lead and make a difference - often quietly, but always meaningfully.

At COHME, supporting and empowering women has been part of who we are from the very beginning. Women make up 95% of our aides and a large part of our leadership and administrative team. This a reflection of the essential role women play in both paid and unpaid care work.

From our founder, Lucy Rosengarten, to leaders like Denise Kellen, Carol Kamine Brown and Laura Radensky, women have shaped COHME at every level - guiding our mission, strengthening our community, and ensuring compassionate care remains at the center of everything we do.

Today, we celebrate the women of COHME and women everywhere who lead with strength, empathy, and purpose everywhere. We see you.
