Caring for someone with dementia can feel overwhelming, but you don’t have to do it alone.

COHME’s new Dementia Support Program provides personalized guidance for families and caregivers, whether or not you have a COHME aide.

How it works:

1️⃣ We begin with a 2-hour in-home assessment (one-time $200 fee) to understand your loved one’s stage of dementia, daily challenges, and overall needs.

2️⃣ From there, families can continue with weekly 1.5-hour in-home sessions ($150/session), focused on practical skills and real-life support. These may include:

- Coaching for family caregivers and home health aides
- Strategies for managing behaviors and adapting the home environment
- Ideas for meaningful enrichment activities
- Clear, written follow-up guidance and resources.

Optional In-Home Enrichment Support (2x per month) is also available, offering creative ways to connect and engage through music, art, pet therapy and nature-based activities - tailored to your loved one.
