Chronic lower back pain is often blamed on discs or muscle strain.

But in some patients, the true cause is a congenital spinal variation known as Bertolotti’s Syndrome.
This occurs when the lowest lumbar vertebra partially fuses or forms an abnormal joint with the sacrum.

Symptoms may include:
• Persistent low back pain on one side
• Pain that worsens with standing or activity
• Limited mobility
• Pain that does not improve with typical therapy

At Vadim Kushnerik Pain Management, proper imaging and diagnostic injections can confirm the source of pain and guide targeted treatment options.

Are you a candidate for spinal cord stimulation?
Spinal cord stimulation may be an option if you suffer from chronic back or nerve pain that has not improved with medications, physical therapy, or injections.

You may be a candidate if:
• You have persistent pain after spine surgery
• You experience nerve-related pain such as burning, tingling, or shooting sensations
• Conservative treatments have failed
• Your pain significantly affects daily life

This minimally invasive therapy works by interrupting pain signals before they reach the brain.

The first step is a detailed evaluation to determine if spinal cord stimulation is right for you.

Lower back pain that does not improve?
Pain that feels deep on one side and radiates into the buttock?
It may not be a disc problem❗️

The sacroiliac joint connects your spine to your pelvis. When it becomes inflamed or unstable, it can cause:
• one-sided lower back pain
• pain when standing up from sitting
• discomfort while walking
• pain that mimics sciatica

Because symptoms overlap with other conditions, SI joint dysfunction is often overlooked.

At Vadim Kushnerik Pain Management, we use precise diagnostic techniques and image-guided SI joint injections to confirm the source and provide targeted relief.

Not all headaches begin in the head.
Sometimes the source is the upper cervical spine❗️

If your headache:
• Starts at the base of the skull
• Feels one sided
• Gets worse after computer work
• Comes with neck stiffness
It may be a cervicogenic headache❗️

Pain medication can temporarily reduce symptoms, but if inflammation in the neck joints or nerves remains, the pain returns.

At Vadim Kushnerik Pain Management we focus on identifying the true source and offering targeted, image guided treatment for longer lasting relief.

Standing for extended periods puts constant pressure on the muscles, joints, and discs in your lower spine. If these structures are already irritated or weakened, the stress builds up and leads to stiffness, aching, and fatigue in the lower back.

Common causes include muscle imbalance, poor posture, disc degeneration, or facet joint inflammation. Over time, the muscles that support the spine become tired, forcing the joints and discs to carry more load, which increases pain.

The good news is that this type of pain is treatable. With the right diagnosis and a personalized pain management plan, it is possible to reduce inflammation, relieve pressure on the spine, and restore comfort in everyday activities.

Cervical radiculopathy happens when a nerve in the neck becomes irritated or compressed. This is often caused by a herniated disc, bone spurs or age related changes in the spine.

Many patients first notice a sharp or burning pain that starts in the neck and travels down the shoulder, arm or even into the fingers. Tingling, numbness and muscle weakness are also common symptoms❗️

Everyday activities like looking at a phone, working at a computer or sleeping in the wrong position can make the pain worse. Over time, untreated nerve compression can lead to persistent discomfort and reduced arm strength❗️

Proper care can help prevent long term nerve damage and improve quality of life.

Chronic back pain often comes from inflammation in the small joints of the spine called facet joints. Over time, arthritis, wear and tear, or injury can make these joints painful and stiff.

Facet joint injections are a minimally invasive treatment that delivers anti inflammatory medication directly into the affected joint using imaging guidance. This helps reduce pain, improve mobility, and make daily movements more comfortable.
Targeted pain management can help you get back to moving with less pain.

Not every headache is a migraine. A common but often missed cause of chronic headaches is occipital neuralgia.
It happens when the occipital nerves at the base of the skull become irritated or inflamed.

Common signs:
• Sharp, shooting pain in the back of the head
• Pain that starts at the base of the skull and travels upward
• Scalp sensitivity or pain when brushing hair

Many people live with this condition without knowing the real cause of their pain.
The good news is that occipital neuralgia is treatable with targeted, minimally invasive procedures such as nerve blocks.

02/12/2026

Sciatica is not a diagnosis on its own. It is a symptom that occurs when the sciatic nerve becomes irritated or compressed.

The sciatic nerve is the longest nerve in the body. It starts in the lower back and travels through the hips, buttocks, and down each leg. When this nerve is affected, pain can radiate from the lower back into the leg, often accompanied by burning, tingling, numbness, or weakness.

Common causes of sciatica include herniated discs, spinal degeneration, muscle tightness, or inflammation around the nerve.

Treating sciatica means addressing the source of nerve irritation, not just masking the pain. Proper pain management focuses on reducing nerve pressure, calming inflammation, and restoring movement to prevent symptoms from becoming chronic.

Many people rest when pain starts. And at first, it helps.
But after days or weeks of limited movement, the body begins to change.

Muscles become weaker and tighter. Joints lose mobility. Circulation slows down.
When you finally try to return to normal activity, the body is no longer prepared for movement, and pain can feel stronger than before.

This does not mean movement is harmful. It means the body needs a safe and guided return to activity.

Proper pain management helps restore mobility, rebuild strength, and reduce pain safely so you can move with confidence again.
