This was a challenging case with an excellent outcome.
Patient had moderate to severe pressure in the whole face for 7 months.
Complex medical history: autism, intracranial surgery 9 months ago, plus
Treatment:
An oral appliance (aka mouthguard)
Trigger point injections with Vitamin B12+Bupivacaine (once a month for 3 sessions)
His prognosis was guarded; he had a very positive frame of mind.
Follow up (4 months since initial evaluation)
Patient only has mild left side pressure
Improved quality of life

Not surprisingly, first drug approved for Obstructice sleep apnea is here. Tirzepatide aka Zepbound has that honor; it’s a huge step for sleep medicine. I wouldn’t be surprised if this became as the first line of therapy for sleep apnea in the near future.

🔴 When should you refer a patient to a TMJ specialist?

If your patient’s TMJ symptoms are severe, persistent, or not responding to typical treatments, it may be time for a referral! Signs to look for include:

⚠️ Chronic pain or difficulty moving the jaw
⚠️ Headaches or migraines linked to jaw discomfort
⚠️ Severe teeth grinding or tooth damage

A TMJ specialist can provide advanced care like joint injections, physical therapy, and even surgery when needed.

Early detection of TMJ disorders = better patient outcomes!

As a dentist, you play a critical role in spotting the early signs of TMJ dysfunction, like jaw pain, clicking, and teeth grinding. Do I need CT scan (or CBCT) to evaluate my airway? This is a common question I am asked.

It certainly provides a ton of information about airway (and TMJ area) but unfortunately it does not add much to clinical information that we get from questionnaire and physical examination.

In our practice, I utilize CT scan if I am suspecting degenerative joint disease in TMJ or suspect a bony growth (benign or malignant tumor etc) or if the clinical picture is atypical.

TMJ issues might be the culprit! Here are some signs dentists should watch for during routine exams:

* Jaw pain or tenderness
* Clicking or popping sounds
* Limited jaw movement
* Chronic headaches or migraines
* Teeth grinding or clenching (bruxism)
* Earaches or ringing in the ears

Magnesium is a key supplement and a therapeutic agent for headaches, fatigue, TMJ disorders, sleep disorders and other ailments. In my practice, I often employ topical Magnesium in certain situations which is very helpful on different fronts.

Magnesium can help different conditions such as headaches, sleep disorders, fatigue, TMJ pain and a few other conditions. In my experience, Magnesium is superior to most drugs when it comes to TMJ disorders (scientific studies are lacking though) and headache disorders.

💪Mg 400-600 mg per day
💪A good trial period is 6-8 weeks; some people find benefit quickly but most people experience benefit after a week or two
💪It is a supplement but it can interact with medications, other supplements and food.
