This saline breast implant had been in place for four decades. Over time the body forms a
capsule of scar tissue around any implant. In long standing implants, that capsule can become thickened, contracted, or even calcified

Interestingly, the implant it self was still intact, but the capsule had hardened significantly
which can lead to firmness, distortion, or discomfort. Breast implants can last many years, but they are not lifetime devices which is why long term follow up is important.

Being his dad has been the greatest privilege of my life.

Most surgeons are afraid to perform a phenol–croton oil peel.

Not because it doesn’t work — but because it requires a deep understanding of skin physiology, wound healing, and careful patient selection. And most of us were never trained how to do it.

When done correctly, however, few treatments in plastic surgery can produce such dramatic improvement in deep wrinkles and severe sun damage.

In this patient, a phenol–croton oil peel was used in combination with a deep plane facelift to resurface decades of photodamage and restore smoother, healthier skin.

A few key principles I keep in mind when performing these peels:

That night he said, “You can do better.”

Challenge accepted.

So I drew Groot again… but this time with a little more effort.

It made me think about something I explain to patients all the time:

Not everyone performing cosmetic procedures is a plastic surgeon. The skillset is not the same.

In many states, almost any licensed physician can call themselves a cosmetic surgeon after taking short courses or workshops.

Board-certified plastic surgeons take a very different path.

• Medical school
• 6–8 years of surgical residency (I did 9!)
• Extensive training in anatomy, reconstruction, and aesthetics
• Board certification in plastic surgery

The difference in training shows up in the details — just like it does in drawing Groot.

When it comes to your face, training matters.

If you’re considering a cosmetic procedure, always ask about your surgeon’s training and board certification.

(And for the record… my five-year-old approved the second Groot.)

I never went on a cruise as a kid.
Honestly… I hadn’t even been on an airplane until I was in college.

Trips like this weren’t part of my world growing up.

Fast forward a few decades and I just spent three days on a Disney cruise with my son.

One of the quiet privileges of working hard and building a life is being able to give your kids experiences you never had.

Watching him take it all in — the ships, the shows, the chaos, the pure excitement — was the best part of the trip.

Back to the OR tomorrow.

But for a few days, the only job that mattered was being his dad.

How plastic surgeons get patients now… apparently.

Step 1:
Give a lecture in front of Harvard diplomas.
Explain anatomy.
Demonstrate surgical technique.

Audience response: 😐

Step 2:
Put on a ridiculous coat.
Lip sync to trending audio.
Make an Instagram reel.

Audience response: 👀

Medicine has always been serious work.
Social media… not so much.

I’ve realized that sometimes people aren’t looking for another formal lecture — they want to see the human being behind the credentials.

So yes, the CV is still ridiculous.
The training is still real.
The surgery is still meticulous.

But every now and then… the fur coat makes an appearance.

Turns out personality and expertise can coexist.

And honestly, that’s probably healthier for everyone.

How I chose plastic surgery.

In medical school I saved my surgery rotation for last… because I was absolutely sure of one thing:

I did not want to be a surgeon.

In my defense, most of the ones I had met up to that point were… intense.

But somewhere during that rotation I realized something.

These were my people.

People who liked fixing problems.
People who liked working with their hands.
People who could see structure, anatomy, and aesthetics all at the same time.

And for someone who grew up loving art and building things, it clicked.

The idea that you could actually fix something in a tangible way was addictive.

I thought surgery wasn’t for me.

Turns out it was exactly where I belonged.

Hot take from a plastic surgeon:

Threads aren’t a facelift.
A facelift isn’t threads.

But that doesn’t mean threads are useless.

I’m 50. I perform deep plane facelifts.
And I just got threads.

Why? Because the right treatment depends on timing, goals, and lifestyle.

Right now I can’t take facelift downtime — I have a young son and a busy operating schedule. Threads give me a subtle lift and a bridge until I’m ready for surgery.

And for the record: absorbable thread lifts don’t prevent future facelifts.

Good aesthetics isn’t about ideology.

It’s about patient selection.
