Happy world anesthesia day!

Some anesthesia providers say the smartest you'll ever be is the day you finish your anesthesia training, but is that really the case? What kind of knowledge do you gain from years of practicing anesthesia, that you can't get from school?

Colby Alexander a GAS CRNA says: "While it may true that the smartest you’ll ever be is the day you finish your anesthesia training, it would only apply if reality stopped at a written exam. In reality, time spent applying learned knowledge solidifies it into muscle memory and cements it into practical application. Experience brings knowledge out of the pages of a textbook and into the real world with real patients with real results."

Being a level 1 trauma center, Abrazo West brings in a diverse array of cases including many high acuity cases. For a significant portion of GAS providers, this is an important factor in their decision to work at Abrazo West. Lucas Kachurick, a CRNA at West says: "I love working at West because of the diverse case mix at varying levels of illness. When I interviewed, I was drawn to the acuity. The ability to handle strokes, traumas, and other critically ill patients was something I required in employment."

Another Abrazo West CRNA, Phillip Bodner, agrees: "The fast-paced environment keeps my skills sharp, and I appreciate the wide variety of cases that allow me to continue growing professionally."

In a new article published this month in Anesthesiology News, Dr. Nathaniel Smilowitz shared new guidelines to help estimate cardiac risk from non-cardiac procedures. The article states:

Patients who need emergency surgery should proceed, Smilowitz said, while management of those who have known cardiovascular conditions, such as heart failure or unstable arrhythmias, should be done by a multidisciplinary team.

To estimate perioperative risk in patients without known heart disease, it is recommended using a validated risk calculator, such as the Revised Cardiac Risk Index (RCRI) or Universal NSQIP Perioperative MI and Cardiac Arrest Risk Calculator.

Next, according to Smilowitz, consider any risk modifiers that would not be detected by calculators, including severe valvular heart disease, severe pulmonary hypertension, elevated-risk congenital heart disease, prior coronary stents or bypass surgery, recent stroke, pacemaker or frailty.

“It doesn’t mean that if you have these things, you are necessarily high risk,” he said, “but it does require you to think about that patient differently than somebody who has no cardiac history.” A person with a pacemaker “may do perfectly well” in the OR, but you may need to change the device settings ahead of time.
