Last month, I had the pleasure of sharing the stage with my dear friend .pedro.rodrigues at the full arch symposium in Dubai organized so well with an amazing lineup by another great friend Hardik Parekh. I wanted to share part of his brilliant lecture about the palatal and transsinus approaches and the biological width.

Based on of the reviewed literature, the concept, formation, remodeling, structure, dimension, and functional significances of periimplant biological width are formed within weeks of implant placement. The formation of biological width around implant is a complex process after several weeks of healing. The biological width around implants is a 3-4mm distance from the top of the peri-implant mucosa to the first bone-to-implant contact or the stabilized top of the adjacent bone, consisting of sulcular epithelium, junctional epithelium and fibrous connective tissue between the epithelium and the first bone-to-implant contact or the stabilized top of the adjacent bone. The biological width forms a biological barrier against the bacteria, influences the remodeling of soft and hard tissue around implant and has implications for clinical aspects of dental implantation. But I have found is it the biological width is often overlooked by clinicians and this can have devastating consequences as long-term.

He was told he would NEVER have implants! After being turned down by multiple places he came all the way from Texas referred by his dentist to the Desert Ridge Oral Surgery Institute in Phoenix, Arizona. Congratulations and many thanks to the colleagues of the Foundation of the Late Professor Alireza Yalda Medical School who have worked so hard with great generosity and love to honor the values of my late wife and comrade, the late benevolent Mrs. Mahvash Salehi, and have remembered the good qualities of Mahvash. I felt it necessary to recount that during Mahvash’s lifetime, she made numerous valuable contributions to several charitable foundations to the opening of schools and to support education of young children in Iran, she always asked me not to let a child go hungry anywhere in Iran.

Medical tourism gone wrong — nerve damage after dental treatment abroad.

A 40-year-old patient returned from Mexico after getting full-mouth crowns. A root canal on tooth #31 led to overextension of sealer into the mandibular canal, leaving him with complete loss of sensation on the right side. Despite tooth removal and debridement, symptoms worsened — including progressive dysesthesia and loss of proprioception.

Microsurgical repair revealed a 1 cm neuroma and nerve destruction. I replaced the damaged segment with a AxoGen® graft after excising the neuroma. Post-op: resolution of dysesthesia and improvement in sensory function.

⚠️ BC sealer in the IAN canal can be neurotoxic and compressive — even biocompatible materials can cause permanent damage when in the wrong place. Don’t just trust anyone with your dental and medical health!

⏱️ Early referral (within 6–8 weeks) is critical for surgical success after a suspected nerve injury.

✈️ One lesson on medical tourism: when complications happen, there’s often no safety net and you are left alone dealing with the consequences.

Access hole size matters !!
Larger access holes compromise the strength of temporary and final restorations, a fact grounded in engineering principles. A 15mm cantilever test using PMMA demonstrates this, where a 200N bite force on a hard object can lead to failure. Recently, I started using the Badger screw and I was impressed that the Badger screw’s design proves resilient in such scenarios with a superior clamping force, and further testing will continue to validate its effectiveness compared to other screws in the market. Can this lead to lower fracture rates in full arch restorations? Badger has the smallest screw channel and I will update you when we are done testing Badger vs other screws.
