Why insurance denials happen more often than patients realize

A major investigative report by ProPublica revealed how one of the nation’s largest insurers has saved millions of dollars by denying medical claims without doctors reviewing patient records.

According to internal documents and former company physicians:
• Claims were denied in seconds, often without opening the patient’s file
• Doctors were instructed to “click and submit” large batches of denials
• Algorithms flagged claims first, and physicians signed off afterward
• Many denials involved tests costing only a few hundred dollars
• The insurer expected most patients would not appeal

Why this matters to patients:

• You may receive a bill after care is already provided
• Denials may have nothing to do with your doctor’s medical judgment
• Many patients pay denied claims simply to avoid the hassle
• Only about 5% of patients appeal, according to internal estimates

Do you know how your pelvic floor muscles are involved in the seggsual response cycle? 🔥

In the first phase the bulbospongiosus, ischiocavernosus, & pubococcygeus contract in order to hold blood in the erectile tissues. For those who have a p***s, this will present as an er****on of the p***s and in v***a owners there will be swelling of the cl****is and l***a minora 🤯

An or**sm💦 will produce rapid muscle contractions of primarily the superficial pelvic floor muscles (bulbospongiosus, ischiocavernosus, and pubococcygeus).

Masters & Johnson found through direct observation🔎 that a p***s owners’ or**sm lasts approximately ✨10-30 seconds✨ while a v***a owners’ or**sm lasts about ✨13-51 seconds✨

Interestingly, the interval of contraction is exactly the same for all genders -once every 0.8 seconds. So, you do the math-it’s a lot.

Overall, Huang concluded, "The differences between the groups were modest, and there was still some possibility that they could have occurred by chance. We believe that our findings suggest that many of the potential benefits of pelvic yoga for women with urinary incontinence may be shared with more general low-impact muscle conditioning exercise."

Over 12 weeks, the average reduction in total UI frequency was 2.3 episodes per day among those in the pelvic yoga program compared with 1.9 episodes per day among those in the general physical conditioning program.
