Here’s what every coder, provider, and healthcare organization should keep top of mind:

✅ Code to the highest level of specificity when supported by the medical record documentation.
📌 ICD-10 is structured in a hierarchical format—each additional digit adds more clinical detail and precision.
📌 Avoid defaulting to unspecified or generic codes when the documentation supports a more detailed code.

💡 CRC Tip: Using MEAT to Improve Risk Adjustment Documentation

💡Accurate documentation is the backbone of risk adjustment coding. One tool that can make a big difference is the MEAT acronym:
M – Monitor: Track the patient’s condition over time.
E – Evaluate: Assess the severity and impact of the condition.
A – Assess: Determine how the condition affects the patient’s care plan.
T – Treat: Document interventions, treatments, and responses clearly.

Using MEAT ensures documentation supports the diagnoses you code, strengthens your risk adjustment submissions, and helps improve quality of care and compliance.
