I recently had the opportunity to join Journey To Launch for a powerful conversation on entrepreneurship, leaving corporate, caregiving, and the inspiration behind my book.🙏🏽

📌One area we touched on that deserves more attention: Advanced Care Planning.

👀Do you know what happens during an advanced care planning visit?

It’s where discussions take place around:
• DNR (Do Not Resuscitate)
• DNI (Do Not Intubate)
• Healthcare proxies
• Living wills
• Medical decision-making authority

These are not easy conversations — but they are necessary.

I am my mother’s healthcare proxy. Because we had these discussions, I was able to advocate, coordinate care, and make informed decisions during some of the most critical moments of her stage 4 cancer journey.

Entrepreneurship requires preparation.
So does caregiving.

📌 Advance Care Planning (ACP) is more than just a conversation; it’s documentation, clarity, and honoring patient wishes.

When we talk about Advanced ACP services, we’re referring to structured discussions between the provider and patient (and often family members) about goals of care, treatment preferences, and end-of-life decisions.

From a coding perspective, remember:

🔹 99497 – First 30 minutes of Advance Care Planning (face-to-face, including explanation and discussion of advance directives such as living wills and healthcare power of attorney).
🔹 99498 – Each additional 30 minutes.

These are time-based codes, so documentation must clearly reflect:

✔ Total time spent in the ACP discussion
✔ That the discussion was voluntary
✔ Who was present (patient, family, surrogate)
✔ Topics discussed (goals of care, resuscitation preferences, life-sustaining treatment, etc.)
✔ Any forms completed or reviewed

If documents such as a living will, DNR, or healthcare power of attorney are signed or updated, that should be clearly documented in the medical record, including that copies were provided or scanned into the chart when applicable.

ACP isn’t just about reimbursement; it’s about patient autonomy, compliance, and ensuring care aligns with values.
