February 7th is National Black HIV/AIDS Awareness Day (NBHAAD), honoring the disproportionate impacts and access to care experienced by African Americans. 🫂

The best remedy is proper education, compassion for the community, and taking steps towards prevention and treatment! 📝

The Clean Slate Clinic is a free resource to help people impacted by the justice system have their records reviewed and possibly sealed, while also connecting the community to reentry resources 🔑.

CCHP provides comprehensive care 🩺 — including primary care, substance use treatment, and more 💙 — and helps you overcome barriers 🚧 so you can get the care you deserve 🌱.

We also assist with connections to housing 🏠, food security 🍎, and job resources 💼 for those who need additional support.

❄️ Due to the snowstorm, only medication dispensing will be available tomorrow, Monday, January 26th. ❄️

If you need your medication pickup tomorrow, please collect it from 7 a.m. - 12 p.m.

All other scheduled appointments for Monday will be changed to telehealth or rescheduled for a different day.

We plan to reopen and continue regular service on Tuesday, January 27th.

Due to the snowstorm this weekend, CCHP will be transitioning to telehealth appointments 💻📞 to help ensure the safety of our patients and staff 🤍

All appointments on Monday will be changed to telehealth or rescheduled 📅. Weather permitting, we will provide essential operations, such as medication pickup 💊.

Please stay tuned for more information on Sunday about our operations 📣. Any updates will come through your Patient Portal, our social media channels 📲, our website 🌐, and the Central Registry for those receiving medication.

We were founded more than 65 years ago to provide accessible community-based comprehensive health services to those in need, regardless of their race, color, national origin, disability, religion, age, s*xual orientation, gender identity, or ability to pay.
