Nasal tip cartilage grows as we age, making tips more prominent and droopy. As a result, rhinoplasty in our older patients often focuses on just making the tip smaller and higher (example 1). Sometimes though, a complete rhinoplasty is in order. This is for those who always thought about it but never got around to it. Results are similar to our young patients, including adding a chin implant for better facial balance in some (example 2).

Many patients say they don’t want a “whole facelift” but a facelift just treats the lower face and neck, the areas shown in this example. It is a powerful tool for correcting the cheek jowl and loose neck though, much more so than a necklift alone. Results look good by two weeks but it takes a full six weeks to heal to the extent shown here.

The more you know the longer these take to do! While symptom relief is the typical motivation for having surgery and is readily achieved, these are true aesthetic procedures. Understanding the subtle nuances in three dimensional skin design and tissue shaping makes all the difference, but it takes decades to figure it all out!

For the surgeons: I love verticals but anything over 500 gm per side and more than Gr. I ptosis is inverted-T territory for me, like this patient.

There are two: great starting anatomy and prioritizing aesthetics over size. Challenging anatomy such as low ni**le position, really tight skin, or a very sharp crease underneath can limit what the surgeon can do. Similarly, trying to “weaponize” the breasts with implants too big compromises overall aesthetics and can even lead to problems requiring further surgery down the road.

For the surgeons: base diameter, IMF anatomy, and patient height the key variables.

Computer simulation can’t capture the subtleties of bone structure and skin quality, nor can it comprehend the range of options available to a surgeon. That’s why I do my own sketches for rhinoplasty patients. Having done hundreds, if not thousands, of “nose jobs,” I think I can give patients a more realistic set of expectations then a computer program can churn out.

Mary Ann being honored by the Skyscraper Museum. Prior award winners include the most important developers and public officials that have worked together to shape our skyline. Her brief bio on the third screen does not include her 9 Deal of the Year Awards and many other accolades. She is the star of this duo!

The event took place at 1 Vanderbilt, New York’s newest and most modern skyscraper next to Grand Central, a structure made possible by Mary Ann’s pioneering concept of re-zoning midtown air rights. The building includes the most amazing observatory on the top 3 floors with the 5th screen showing the transparent floor balcony. This highly interactive observatory is called The Summit. Not to be missed!

Our lovely patient is seen 6 weeks after rhinoplasty, chin implant, and liposuction of the neck. It is so important to move the chin forward not just for better facial balance but also so the muscles that push the lips together do not have to strain from mechanical disadvantage. The lips now look more relaxed. A pleasing neck contour has also been established with the aid of liposuction. All of this shows on the front view too!

For the surgeons: The deeper labiomental crease is a good trade in this case for the much improved projection. A touch of supratip edema at this time interval is expected.

Our male patients most commonly seek facelifts to remain competitive in the workplace, most notably in the digital industry. This procedure, which included a chin implant, eliminated the downward curve and backward sweep of the face and neck that blurred the distinction between the two. A more pleasing rectilinear configuration has been established with right angles at the chin and middle of the neck where a hidden “Adam’s apple” is revealed.

For the surgeons: TXA in the local has been just as effective in males as females so intravenous use has proven to be entirely optional.

For the surgeons: Either extra-small or small curvilinear chin implant through a 5 mm submental incision works great! Front view lip strain relief just as important a gain.
