We put children and families first, prioritizing the total wellbeing of Medicaid eligible children who have faced significant challenges to their physical,

The CCF is at the forefront of a state-wide movement to transform the way that high-need children and families access critical healthcare and wellness services. Navigating the old health care system is difficult for Medicaid recipients with high-cost and complex conditions that drove frequent hospital visits. A significant percent of Medicaid spending is dedicated to this high-need subset of the Medicaid population. Appropriately accessing and managing these services, through improved care coordination and service integration, is essential in controlling future health care costs and improving health outcomes for the most vulnerable New Yorkers. The Health Home care management service model allows all of an individual’s caregivers to communicate with one another so that all of a patient’s needs are addressed in a comprehensive manner. Health Home Care Managers are better able to assure that the most vulnerable and medically-fragile Medicaid recipients receive everything necessary to stay healthy, out of the emergency room, and out of the hospital. The New York State Department of Health designated the CCF as a pediatric Health Home in June 2015. We begin providing care management as a Health Home in 2016.

Now Accepting Referrals for SCN Screening through CCF!

CCF is accepting referrals for Health Related Social Care Needs (HRSN) Screenings to connect Medicaid patients/clients with community-based social care services through the SCN Network Public Health Solutions.

Who should be referred?

Individuals with identified social needs, including:

Housing instability

- Food insecurity

- Transportation barriers

- Utility assistance needs

- Other health-related social needs

What happens after referral?

CCF completes an SCN eligibility screenings!

Eligible individuals are connected to Health-Related Social Needs (HRSN) services

Ongoing care coordination may be provided through the SCN

How to refer:
