This lovely patient was interested in lifting her nasal tip without extensive surgery. The shape of her nasal tip and the proportions of the rest of her nose were quite good so we opted to avoid opening and dismantling the whole nose and instead went for a closed rhinoplasty limited to the nasal tip under local anesthesia. The exact technique is what I call a Tip Stitch®️. As you can see, this approach adds support to the otherwise nicely shaped tip, bringing it into a better position in less than 45min, without much of the recovery of a full surgery.

The initial result has a slight overcorrection but settles into place as the swelling goes down. While this is a surgery, it is limited in nature which allows for quick downtime. In the right patient it can be a better option than fillers placed off label into the nosal tip.

Anyway, we are very pleased with the results so far and love being able to offer an option which allows people to get back to their busy lives quicker.

Happy early patient result from an Endoscopic Midfacelift + Deep Plane Composite Facelift. We love how the lifted cheeks create contour and a more cheerful appearance.

Restoring your own fat pads back up to a youthful position can eliminate the need for fat injections and avoid an overly round face.

Many facelifts will only focus on the lower face and neck and lose sight of the midface.

The endo midface lift goes in a deeper plane avoiding many of the risks associated with previous surgery, fillers, threads, and energy devices. This makes it an attractive option if these factors are at play.

Ready for a change?

This upbeat patient is two weeks post op from an endoscopic midface lift plus revision facelift surgery. She was bothered by the accumulating loose skin of the face and neck as well as the cheek dropping over time. Previous facelift surgery and thread lifts did not hold up over the years and it was time for a refresh. I performed an Endoscopic Midface Lift, Deep plane revision Facelift, Necklift and Chin Implant. I like the endoscopic midface lift as it really can elevate the cheeks and lifts from the deepest layer of the face away from previous scarring and threads. The modest chin implant elongated the jawline beyond what is possible with tightening alone. She was back to work within a week and has some mild residual bruising. We love the initial result and positive attitude.

Manifesting the morph into reality. Previous nasal surgery left this lovely patient with alar retraction, midvault collapse, and an unsupported droopy nasal tip. The nostril being pulled up gave off a 'snarl' / angry appearance that did not match her normal effervescent self. To make matters worse, her nasal breathing was compromised and there was no cartilage inside the nose to use in surgery. The images above are the before, the goal, and the actual outcome. I like to use 2D imaging to see what changes make patients happy. It takes away some of the guesswork of what they are imagining in their head. In terms of the surgery, it was an open approach rhinoplasty with rib cartilage for grafting to re-support the structure. Form and function restored and all js well. Very special gift from a happy patient. It means a lot for someone to take the time to paint this impressively accurate portrait. I love the concept of leaving things better than than you find them. Lucky for me, my work allows me a chance to impact peoples' lives. This gift is a nice reminder of that. ..Now to hang it in the break room, like a communist dictator, and spook our staff.

Nasal tip so uplifting, your therapist will call it for advice. ❤️👃🏻

This patient had an to address multiple parts of his nose, but was mainly bothered by the droopy nasal tip. Nasal tip support is extremely important to maintain the harmony and longevity of the nose.

We had a very special visitor to the office, my fellowship director and mentor . Coming all the way from in Beverly Hills where I trained. We discussed all things rhinoplasty, preservation rhinoplasty, and even "preservation" facelifts-- which is a variation of the extended deep plane composite lift developed by Frank Kamer at Lasky Clinic decades ago. There is no East Coast - West Coast conflict when it comes to surgery.

Sometimes the best results are from what's not* done. 🔍

Thank you to for featuring me in this article. IMO the best way to avoid dissolving fillers is to be mindful before starting.

Ask questions.
Is your injector qualified❓
Are they using the correct product for the area❓
Is too much product being place ❓
Is it placed too often ❓
Is surgery a better option for the result you want ❓
