The Caregiver Program of SNAP

The Caregiver Program of SNAP Providing support for informal caregivers with services at home, in the community, and by phone. We serve Queens Community Districts 8–14.

Respite • Supplemental Services • Individual Counseling • Support Groups • Educational Workshops • Case Assistance Hours of operation have changed due to Covid-19. The office is open from 8:30am to 4:30pm Monday, Wednesday and Friday. We service Community Boards 8, 9, 10, 11, 12, 13 and 14. To join the program call us at 718-527-5380. We will do a simple intake over the phone and then a case worker will come out to meet you and your love one in the home. Due to Covid-19 assessments can be done over the phone. At that point you can take advantage of any or all of the services listed. We look forward to working with you and your loved one.

Today we raise awareness about Parkinson’s disease and recognize the importance of education, support, and community for individuals and families affected by this condition.

An estimated 65,000 New Yorkers are living with Parkinson’s disease, highlighting the importance of accessible resources, supportive programs, and caregiver support in our communities.

Caregivers, loved ones, and community networks play an important role in helping individuals navigate Parkinson’s while maintaining quality of life.

Poetry therapy uses reading, writing, and storytelling to support emotional processing and self-reflection.



Putting feelings into words can be incredibly healing — especially for caregivers carrying so much internally. Writing or sharing stories helps people feel understood, connected, and less alone.



At SNAP, journaling prompts, storytelling circles, and reflective activities offer caregivers space to express their experiences and find strength in community.

Music therapy uses singing, listening, rhythm, and instruments to improve emotional, cognitive, and physical well-being.



Music has a powerful way of reaching memory and emotion — especially for individuals living with dementia. A familiar song can spark recognition, joy, and connection in seconds.



At SNAP, music shows up in sing-alongs, rhythm groups, and live performances that:

Dance/Movement Therapy uses movement, body awareness, and nonverbal expression to support emotional, cognitive, and physical well-being.



Caregiving can live in the body as tension, fatigue, and stress. Movement helps release that — improving mood, balance, and connection.



At SNAP, movement-based programs help:
