The Caregiver Program of SNAP

The Caregiver Program of SNAP Providing support for informal caregivers with services at home, in the community, and by phone. We serve Queens Community Districts 8–14.

Respite • Supplemental Services • Individual Counseling • Support Groups • Educational Workshops • Case Assistance Hours of operation have changed due to Covid-19. The office is open from 8:30am to 4:30pm Monday, Wednesday and Friday. We service Community Boards 8, 9, 10, 11, 12, 13 and 14. To join the program call us at 718-527-5380. We will do a simple intake over the phone and then a case worker will come out to meet you and your love one in the home. Due to Covid-19 assessments can be done over the phone. At that point you can take advantage of any or all of the services listed. We look forward to working with you and your loved one.

Young-onset dementia is often misunderstood, and myths can delay families from getting the support they need. This month, we’re helping raise awareness by sharing facts that bring clarity, compassion, and hope.

People under 65 can and do experience dementia, and their journeys often look very different from those diagnosed later in life. Early changes in memory, behavior, or problem-solving aren’t “just stress” — they deserve to be taken seriously.

By learning the facts, we can reduce stigma, recognize signs sooner, and connect families to resources that make a real difference. Awareness empowers community.

Not all disabilities are visible.

Today on International Day of Persons with Disabilities, we honor the many individuals living with cognitive changes, sensory differences, mental health conditions, chronic illnesses, and other disabilities that aren’t always seen, but deeply impact daily life.

At SNAP, we’re committed to creating a community where every person feels understood, valued, and supported. Compassion, patience, and awareness go a long way in building an inclusive Queens for all. 💙

Let’s continue learning, listening, and making space for everyone’s unique experiences.
