Our Story

Community Care is a comprehensive agency offering a variety of clinical and social support services to meet patient needs in the community. Our specialty services include the Maternal-Child Health and Behavioral Health, both tailored to the needs of patients with greater risk of hospitalization. Upon the initial community visit, our staff assess the patient and develop an appropriate plan of care to meet the patients' needs and health concerns. NYC Health + Hospitals/Community Care, which includes more than 800 dedicated employees, reaches outside the walls of our hospitals and brings health care to patients where they live. Services include home care, community care coordination, complex care and community care management.
