Medicaid isn’t a line item. It’s a lifeline.

As that LARGEST health insurance program in the United States, Medicaid provides vital coverage – that otherwise would be out of reach – to over 68 million people. This coverage includes prenatal, deliver, and postpartum care. Some states also use their own funds to cover abortion and gender affirming care.

Midwifery provides vital patient-centered care and support throughout pregnancy, childbirth, and the postpartum period — in safe, healthy, and comfortable environments.
Yet in Georgia, midwives are unfairly subjected to extreme restrictions that prevent them from practicing freely. This is why midwives are suing the state of Georgia.

These restrictions include requiring all nurse-midwives to have formal, written agreements with physicians (which results in midwives having to make monthly payments) and requiring all midwives to have nursing degrees. These regulations are designed to make it harder for midwives to practice, making Georgia one of the country’s most restrictive states for midwives.

Due to years of policy failure, more than one‑third of the state’s counties – especially those with rural Black communities — are maternity care deserts. Midwives help more birthing people access safe joyful care and can improve health outcomes. Lawmakers should be embracing and uplifting midwives, not impeding them.

“Bodily autonomy does not go out the window because you are pregnant!”

In 2024, Cherise Doyley, once a doula herself, was taken to court mid-labor for refusing to agree to a C-section unless there was an emergency. Doyley’s case is not a singular experience — judges have ordered pregnant people to have c-sections against their will for decades. In the state of Florida, pregnancy is the ONLY condition where courts have ruled that a patient can be FORCED to undergo unwanted treatment.

Black pregnant people deserve to be listened to, believed, and treated with respect and dignity. That’s why NIRH is working to support access to community-based, culturally reflective midwives.

Dr. Joia Crear-Perry, a founding Steering Committee member of the Black Mamas Matter Alliance (BMMA), reminds us that treating people differently based on race and gender is a choice, which means our elected leaders can, and must, choose differently. New choices can improve how systems are built and how care is delivered – so Black birthing people are respected and able to make informed decisions every step of the way.

Birth Justice policies improve Black maternal health outcomes in the United States by increasing access to the resources and care they need to have a joyful and culturally aligned pregnancy and birth. They look like securing funding for Black midwives and doulas, increasing Medicaid coverage to ensure birth work is sustainable and accessible, and establishing rights for birthing people.

Organizations like BMMA are leading this work – centering Black voices and experiences to advance solutions rooted in equity and justice. Follow Black Mamas Matter Alliance for new resources, event details, and ways to get involved during !

This week's ruling in Louisiana may have paused the state's attempt to limit how people can get medication abortion, but the fight is far from over.

The Court is giving the Trump Administration the next six months to move forward with its FDA “review” of mifepristone — one of two medications commonly used together for medication abortion — that we expect will rely heavily on junk science and will be nothing more than a thinly veiled attempt to lay the groundwork for additional nationwide restrictions.

Today, more than 1 in 4 people who have abortions do so through telehealth. It increases access by offering remote, patient-centered care that reduces travel, costs, and stigma, allowing people to manage care on their own terms. Leading medical experts agree that restricting telemedicine would have no safety benefit and would only create barriers to care, which is exactly what this administration wants.

As these political attacks on medication abortion continue, NIRH is dedicated to working with our partners to increase access and debunk misinformation.
