สุภาพสตรีท่านหนึ่ง หกล้มและบาดเจ็บบริเวณข้อเท้าในขณะที่พาสุนัขออกไปเดินเล่น
ขณะที่แพทย์ได้ตรวจข้อเท้าที่บวมของเธอนั้น ได้สังเกตเห็นความเคลื่อนไหวในบางทิศทางที่ไปได้อย่างจำกัด และอาการของการหมดความรู้สึกในบางส่วนของขาและเท้าของเธอ

This case highlights the importance of thorough physical examination. A women fell and injured her ankle while walking her dog. The attending physician examined her swollen ankle and noticed limited movement in some direction and diminished sensation on some parts of her leg and foot. As usual I like to do these medical tutor videos in English language since English is the universal language of medical education, in addition board exams such as the USMLE are in English language right? So, it is extremely beneficial to have a good grasp of the English language. I know a few of you have scheduled your step exam and I want to wish you the best of luck! Believe in yourself, you have worked hard for this and have encountered thousands and thousands of practice questions. Manage your time wisely during the exam, pace yourself well and I guarantee, you will be more than fine!
What is the content of this video:

Firstly, I emphasized that in medical studies, it is essential to be able to apply the knowledge that you have acquired. Any fool can memorize and regurgitate the information, but on the exam, you only get points of you are able to apply the knowledge correctly. I gave a short example of applying the knowledge and how some questions can trick you.

Then I moved on to explaining the mechanism of inflammation which is a natural immune response (a student asked me about inflammation a few weeks ago).

Finally, we went through a USMLE style question where I shared my train of thoughts as I go through the question. Today’s question was an endocrinology question. One highlight that I want to make is the relationship between Dopamine, Prolactin and Gonadotropin Releasing Hormone. This concept is often tested on your step exam especially psychiatric medicine section.

1. ซื้อยากันยุง ทำไมเราต้องซื้อยากันยุงเพราะว่ามีอะไรอยู่ในอาหารที่เป็นนำ้? สิ่งสกปรกต่างๆเช่น เศษอาหารที่มาจากสัตว์,นำ้เสีย ,นำ้มัน,กาซ ,เศษอุจจาระ และอื่นๆ ยุงชอบของที่ไม่สอาดในพื้นเปียกชื้น ดังนั้นบริเวณที่นำ้ท่วมจะเป็นแหล่งเพาะยุงที่ดี ยุงเป็นพาหะ ที่ทำให้เกิดโรคต่างๆเช่น มาเลเลียและไข้เลือดออก ดังนั้นถ้าเราใช้ยากันยุงเราอาจจะสามารถป้องกันตัวเราจากโรคบางชนิด เช่นมาเลเรียและไข้เลือดออก
2. อยู่ในที่แห้ง อย่าพยายามไปอยู่ใน ที่นำ้ท่วมเพราะว่ามีแบคทีเรียหลายชนิดอยู่ในนั้น ถ้าหากคุณต้องสัมผัสนำ้ท่วมให้ล้างบริเวณร่างกายที่สัมผัสนำ้ท่วมด้วยสบู่และนำ้ขวด ขอให้แน่ใจว่านำ้ดังกล่าวจะไม่โดนใบหน้าของคุณเพราะแบคทีเรียจะเข้าสู่ร่างกายคุณโดยผ่านทางตา จมูก และปาก นี่คีอสิ่งที่ดีที่สุดในการป้องกันตัวเอง คืออย่าไปอยู่ในที่มีนำ้ท่วม
3.ใช้นำ้ขวดสำหรับดื่มและชำระร่างกาย
4. จัดหาพลาสเตอร์ปิดแผลชนิดกันนำ้เอาไว้ไก้ลๆมือ ถ้าคุณมีแผลเล็กๆจงล้างแผลด้วยสบู่และนำ้ขวด ใช้ขี้ผึ้งปฏิชีวนะเช่น Neosporinและปิดแผลด้วยพลาสเตอร์ปิดแผลกันนำ้ สำหรับแผลใหญ่ลึก ติดต่อเจ้าหน้าที่ สาธารณสุข เพื่อดูแลรักษา
5. สวมใส่เครื่องแต่งกายที่เหมาะสมถ้าต้องเดินผ่านบริเวณที่มีนำ้ท่วม ถ้าคุณเป็นเจ้าหน้าที่ทางราชการหรือปัฏิบัติหน้าที่ช่วยเหลือในเวลาฉุกเฉินต้องเดินผ่านบริเวณนำ้ท่วมคุณจะต้องสวมใส่รองเท้าบูท ถุงมือยาง กางเกงขายาว เสื้อแขนยาว และแว่วตานิรภัย
6. จัดหาเครื่องแต่งกายสำรองที่แห้งเอาไว้ไกล้ตัว หากเครื่องแต่งกายของคุญเปียกเพราะนำ้ท่วมและคุณไม่มีเครื่องแต่งกายที่แห้งเพื่อมาเปลี่ยนคุณจะเสี่ยงต่อการเป็นไข้(Hypothermia)ได้ ซึ่งจะเกิดจากอุณหภูมิในร่างกายลดลงทำให้หนาวสั่น มึนงง ดังนั้นเครื่องแต่งกายสำรองจึงมีความจำเป็น กรุณาระวังตัวรักษาตัวเองให้ปลอดภัย
