In 2014, Staten Island Performing Provider System (SI PPS) formed an integrated network of medical, behavioral, and social services agencies under the New York State Department of Health Delivery System Reform Incentive Payment Program (DSRIP). Our founding goals included improving the quality of care, reducing costs and improving health outcomes for Staten Island’s Medicaid and uninsured populations. By the end of the DSRIP Program, we exceeded our goals and maintained our network to continue enhancing and refining the transformative work built in the Staten Island Community. Today, we continue to work with our partners to improve population health outcomes, address social determinants of health, grow our network and reduce health disparities. We are dedicated to improving health equity by holding conversations with our community, creating educational and workforce opportunities for youth, and bridging connections with non-traditional service providers to meet people where they are in the community.

Our members with asthma are often left speechless when they find out just how much support they can get for FREE if they have Medicaid.

Asthma is a common, but serious condition if left untreated. Luckily, many of the treatments are simple and can be accessed at no-cost through the Social Care Network. Eligible Medicaid Members can get connected to life-changing resources through the SCN for asthma support, nutrition, housing support, and so much more.

Cold, dry air can worsen symptoms and increase risk of asthma attacks for vulnerable groups. Here’s how we can help!⬇️

Social Care Network navigators are here to help assist you in finding resources to manage your asthma. If you have Medicaid, the SCN can connect you with services such as:

🏠Home remediation (air filter repair, plumbing repairs, waterproofing, etc.)

🐀Mold and pest control

🧽Asthma supplies (microfiber cloths, non-irritating cleaning supplies, protective bedding, and more)

Talk to your doctor to learn more about managing your asthma and creating an effective asthma action plan.
