Partners Health Plan is a New York based non-profit organization that is proudly offering the first managed care program designed exclusively for Medicare and Medicaid eligible individuals with intellectual and developmental disabilities in NYC, Long Island, Rockland, and Westchester. PHP Care Complete (Medicare-Medicaid Plan), Your Plan, Your Way! PHP Care Complete FIDA-IDD Plan offers all medical, dental, behavioral health, Part D, and long-term supports and services covered by Medicare and Medicaid, including OPWDD services. The actual benefits and services that a participant receives is based on his or her assessed needs and personal preferences. Each participant will have a Clinical Team Leader and Care Manager assisting in the development of a customized Life Plan that meets your personal goals and valued outcomes.
