When Queens Home Care Branch Director Pearline realized that one of her patients didn’t have the support he needed at home, she took matters into her own hands. “The support he had wasn’t enough for us to do our job safely, so I committed to partnering with these outside organizations [the patient’s health plan and visiting doctor’s office]. If we couldn’t meet these needs, he should go back to a skilled nursing facility. But he didn’t want that — and we didn’t want it, either.”
